The goal of the Eskenazi Health Infectious Diseases Clinic (IDC) is to improve the health and lives of individuals living with or at risk for infectious diseases through comprehensive, patient-focused care The clinic provides quality care to all, regardless of age, race, religion, gender, beliefs, culture, mental or physical ability, sexual orientation, or gender identity. The IDC interdisciplinary team of care coordinators, doctors, interpreters, nurses, nurse practitioners, pharmacists and social workers partners with each patient to provide the quality care through individualized treatment plans, education, and access to clinical trial research.
In addition to medical care, the IDC offers care coordination services sponsored by the Indiana State Department of Health and the Marion County Public Health Department. Social workers and care coordinators work with people living with HIV and their significant others to prevent interruption of medical care and to facilitate healthy living by linking clients to non-HIV medical care, emotional support services, financial assistance, medical insurance, transportation and other community resources.
The IDC travel team brings all the specialty services provided at Eskenazi Health's downtown campus to several community health centers throughout Indianapolis, which are listed below. This team also provides prompt linkage to care for patients diagnosed through routine HIV screening in the health centers. Appointments with this team can be scheduled by calling 317.880.3500.
