Patients have the right to choose a post-acute care provider from among the facilities capable of accepting their referral and meeting their post-acute needs. But, after a hospitalization, choosing the right provider can be overwhelming. Insurance plan coverage, quality of care, location and more are all factors in the selection.
To help with this choice, Eskenazi Health makes available a list of providers who may be able to provide post-acute care services. The list includes select skilled nursing facilities and home health agencies who Eskenazi Health works closely with in the Eskenazi Health Post-Acute Care Collaborative. These facilities and agencies were chosen because they meet certain quality and clinical criteria, and they are willing to work with Eskenazi Health to improve the quality of care patients receive, while communicating closely with the hospital.
Providers participate in the Post-Acute Care Collaborative on a voluntary basis. Providers do not pay a fee to be included. Patients are not required to select a provider from the Post-Acute Care Collaborative. A list of other Medicare certified home health agencies, skilled nursing facilities, inpatient rehabilitation facilities or long-term acute care hospital providers or facilities is available.

Patients should be aware that some insurance companies have contracts with specific providers, suppliers and facilities. This may affect the post-acute provider or service patients can use. If they choose a post-acute provider or service that is not covered under their insurance plan or not participating in Medicare, they could be liable for paying the costs involved.
Patients enrolled in a managed care organization should check whether their chosen provider is covered by their insurance plan before selecting them.
