There are many different health care coverage programs available. What do you qualify for? What’s the right coverage option for you and your family?

The Eskenazi Health Financial Eligibility Services team is here to help you explore your options and ultimately apply for the program that is best for you. Available health care coverage programs include:

  • Federal Marketplace
  • Indiana Coverage Programs (IHCP)
    • Hoosier Healthwise
    • Children's Health Coverage Program (CHIP)
    • HIP 2.0 Healthy Indiana Plan
    • Medicaid for the Aged, Blind and Disabled - M.E.D. Works and Behavioral & Primary Health Care Coordination (BPHC)
    • Medicare Savings Programs

    • Family Planning
    • Breast and Cervical Cancer Program (BCCP)

Eskenazi Health provides primary care through the Eskenazi Health Center system, which has multiple locations throughout Marion County. Eskenazi Health Center is a Federally Qualified Health Center and does not deny services to any individual based on inability to pay; it is open to all. All Eskenazi Health Center locations have a sliding fee schedule that bases discounts on family size and income. There is also an optical shop sliding fee schedule.

Financial counselors also can give referrals for other payor resources to qualified patients. Counselors are available at Sidney & Lois Eskenazi Hospital, Monday - Friday, 8 a.m. - 6 p.m. Please see individual Eskenazi Health facilities listed in the locations tab below for their hours.

Appointments are currently needed to meet with a financial counselor. Appointments can be made by calling 1.855.202.1053 or scheduled online through Eskenazi Health MyChart. Login to your Eskenazi Health MyChart account or visit the MyChart page to learn more. Patients in need of food assistance are encouraged to visit the Food as Medicine page to learn more about available programs including information about SNAP application assistance.

Eskenazi Health would like to remind all patients covered by Medicaid to visit FSSABenefits.IN.gov to report any changes and check eligibility. Beginning in April 2023, eligibility redeterminations will resume, with a plan to return to normal operations within 12 months. Many redeterminations will be done automatically based on information the state has available. However, in some situations, the state of Indiana will need to ask the member for information about themselves and their family, such as current address, employment status and income, age, and family size. Patients with questions or concerns about eligibility redeterminations may contact a member of the Eskenazi Health Financial Eligibility Services team by calling 1.855.202.1053 or scheduling an appointment online through Eskenazi Health MyChart.

What to Bring to a Financial Counseling Visit

Below is a general list of items patients may need during the application process. For questions about whether a document is necessary for a specific application, please call a financial counselor at 1.855.202.1053. Follow the following prompts: Press #1 for English or #2 for Spanish. Then, press #2 to speak to a financial counselor.

  • Photo identification
  • Address verification (e.g., information from utility bills)
  • Employer and income information for every member of your household for the past 30 - 90 days (e.g., information from pay stubs or W-2 forms)

Other items and documents may be requested by the financial counselor on or prior to your appointment date.
