Eskenazi Health Center Discounted Services Notice to Patients
Eskenazi Health Center Westside has been designated an Age-Friendly Health Systems – Committed to Care Excellence health center by the Geriatrics Workforce Enhancement Program, a grant program under the Health Resources & Services Administration, for integrating age-friendly care into the primary care environment. Age-friendly practices are based on implementation of the four Ms, What Matters, Medications, Mentation, Mobility, which are essential elements to the care of older adults. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States.

Eskenazi Health Center Primary Care – Adolescent Program
Recognizing that teens have distinct and diverse needs, Eskenazi Health Center provides comprehensive health services for them – from routine health care to treatment of emergencies and chronic illnesses. Services include care for acne, sports-related injuries, menstrual issues and general illnesses in addition to mental health screenings, smoking cessation programs and immunizations. Senior Primary Care Services
On Thursday afternoons from 1 – 5 p.m. a geriatrician (doctor who specializes in the care of older adults) provides consultations for patients 65 and older who want to keep their primary care provider but would like a complete health and physical examination conducted by a team specializing in the care of older adults. The team develops a personalized plan of care then reviews recommendations with the patient, family members and primary care providers.
Patients must have a referral from their Eskenazi Health primary care provider. Reasons to refer any established older adult Eskenazi Health patient for a consultation include:
- Abuse/mistreatment
- End-of-life planning
- Falls
- Functional loss/frailty
- Home safety
- Incontinence
- Malnutrition/weight loss
- Memory concerns
- Mobility issues
- Taking multiple medications
