The Eskenazi Health Medical-Legal Partnership, Faegre Baker Daniels, and Eli Lilly and Company are inviting Eskenazi Health patients to a free Advance Care Planning Legal Aid Clinic on Tuesday, April 29, from 10:30 a.m. – 2:30 p.m. in the Faegre Baker Daniels Conference Room located on the first floor of the Eskenazi Health Outpatient Care Center. Spanish interpretation services will be available.

Sponsored by Eskenazi Health Foundation, the event will provide patients with advance care planning assistance. Advance care planning involves making decisions about preferences for future care in the form of a legal document. It also gives patients the opportunity to choose someone to make medical and financial decisions if they become unable to do so. This planning often makes difficult decisions easier for loved ones because the patients’ wishes are known.

Lawyers from Faegre Baker Daniels and Eli Lilly and Company will be on site and may be able to help Eskenazi Health patients with:

• Drafting a will which allows patients to plan for their children
• Appointing a financial power of attorney
• Appointing a health care power of attorney and a health care representative
• Creating a living will

On April 29, lawyers may be able to make visits to patients’ hospital rooms upon request. During the event, please call 317.777.9199.
