The Center for Youth & Adults with Conditions of Childhood (CYACC) works to improve the move from pediatric to adult services for youth with special health care needs. CYACC provides an in-person consultation and subsequent care coordination during this transition. Clinical visits are offered at Eskenazi Health Center West 38th Street. Eskenazi Health Center West 38th Street is a federally qualified health center which offers a sliding fee discount program based on household size and income and is open to all regardless of ability to pay.
Families, primary care providers and specialists can refer youth between the ages of 11 and 22 who have special needs (such as serious chronic health conditions, intellectual disability, physical disability and/or serious mental illness) for help with their transition needs. CYACC’s multidisciplinary care team includes physicians, nurses, social workers and other professionals, many of whom have personal experience as a parent of a person with special health care needs.
The team works with patients and families to create medical summaries that can be easily transferred among medical providers and shared plans of care with action items to meet transition goals and improve health care for individuals with complex health needs. Learn how to be active participants in their care by working with their primary care and specialist teams

Establish sustainable health care insurance

Determine the appropriate support people for medical decision-making

Obtain the appropriate government support services
Patients work on caring for their own health needs, while also receiving assistance in meeting those needs. For example, CYACC helps patients apply for Medicaid services waivers, obtain legal referrals for power of attorney and guardianship, and work with special education and vocational rehabilitation services.
