Eskenazi Health Center West 38th Street has been designated an Age-Friendly Health Systems – Committed to Care Excellence health center by the Geriatrics Workforce Enhancement Program, a grant program under the Health Resources & Services Administration, for integrating age-friendly care into the primary care environment. Age-friendly practices are based on implementation of the four Ms, What Matters, Medications, Mentation, Mobility, which are essential elements to the care of older adults. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States.

CYACC provides an in-person consultation and subsequent care coordination during this transition. As part of the federally qualified health center, CYACC offers a sliding fee discount program and is open to all regardless of ability to pay.
Senior Primary Care Services
On Friday mornings from 8 a.m. – 12 p.m. a geriatrician (doctor who specializes in the care of older adults) provides consultations for patients 65 and older who want to keep their primary care provider but would like a complete health and physical examination conducted by a team specializing in the care of older adults. The team develops a personalized plan of care then reviews recommendations with the patient, family members and primary care providers.
Patients must have a referral from their Eskenazi Health primary care provider. Reasons to refer any established older adult Eskenazi Health patient for a consultation include:
- Abuse/mistreatment
- End-of-life planning
- Falls
- Functional loss/frailty
- Home safety
- Incontinence
- Malnutrition/weight loss
- Memory concerns
