Eskenazi Health Center Primary Care – Adolescent Program
Recognizing that teens have distinct and diverse needs, Eskenazi Health Center provides comprehensive health services for them – from routine health care to treatment of emergencies and chronic illnesses. Services include care for acne, sports-related injuries, menstrual issues, general illnesses, mental health screenings, smoking cessation programs and immunizations. Legacy House Program
Legacy House Program provides trauma-informed care and compassionate support to adults and children affected by all types of violent crimes including child abuse, domestic violence, sexual assault, robbery and stalking. Healing is aided through intervention, support and understanding.
Legacy House Program provides a full range of services including individual and family counseling, crisis intervention, victim advocacy, community education, violent crime compensation information and assistance, temporary emergency protective orders, shelter and social service referrals, and home address confidentiality information and assistance.
Services are available to patients regardless of ability to pay. Patients without insurance can meet with an Eskenazi Health financial counselor to determine eligibility for insurance and the sliding fee schedule which bases discounts on household size and income.

Senior Primary Care Services
On Tuesday afternoons from 1 – 5 p.m. a geriatrician (doctor who specializes in the care of older adults) provides consultations for patients 65 and older who want to keep their primary care provider but would like a complete health and physical examination conducted by a team specializing in the care of older adults. The team develops a personalized plan of care then reviews recommendations with the patient, family members and primary care providers.
Patients must have a referral from their Eskenazi Health primary care provider. Reasons to refer any established older adult Eskenazi Health patient for a consultation include:
- Abuse/mistreatment
- End-of-life planning
- Falls
- Functional loss/frailty
- Home safety
- Incontinence
- Malnutrition/weight loss
- Memory concerns
- Mobility issues
- Taking multiple medications
