How To Sign Up
Eskenazi Health patients can sign up online at MyChart.EskenaziHealth.edu. If you recently visited an Eskenazi Health location, please look for your MyChart activation code on your After Visit Summary paperwork. If you do not have an activation code, you can create an Eskenazi Health MyChart account by entering information that matches your medical records. You can use the Quick Start Guide to help you get started.
